Access Services is seeking a highly motivated and compassionate Homeless Outreach Worker to join our team in Plymouth Meeting, PA.

Job Summary:

We are looking for a dedicated professional to work with individuals and families experiencing homelessness, providing support and connecting them with essential resources. As a Homeless Outreach Worker, you will play a vital role in our team's efforts to address homelessness and promote housing equity.

Key Responsibilities:
  • Engage with individuals and families in need, building trust and rapport to facilitate access to support services.
  • Conduct assessments and screenings to identify individuals' needs and develop personalized plans for support.
  • Collaborate with community partners to ensure seamless service delivery and maximize resources.
  • Participate in on-call rotations to provide 24/7 support and respond to emergencies.
  • Document case notes and outcomes in our electronic health record system.
Requirements:
  • Bachelor's degree in social work, human services, or a related field.
  • Experience working with vulnerable populations, preferably in a homeless services setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals and groups.
  • Ability to work independently and as part of a team, with a focus on collaboration and problem-solving.
  • Proficiency in technology, including electronic health records and other software applications.
