About the RoleAs a key member of our interdisciplinary team, you will utilize your advanced critical thinking skills to develop, implement, and evaluate patient care plans. Key Responsibilities- Develop and maintain accurate patient records, detailing treatment plans, progress notes, and medication administration.
- Conduct thorough patient assessments, identifying areas for improvement and adjusting care plans accordingly.
- Liaise with healthcare professionals to facilitate seamless care transitions and optimize patient outcomes.
- Promote a culture of respect and inclusivity, fostering positive working relationships among team members.
- Actively participate in quality improvement initiatives, analyzing data to inform best practices and advance patient care.

The estimated annual salary for this position is between $65,000 and $90,000, reflecting the complexity of the role and the level of expertise required.
