About the Role:
As a Clinical Manager at CenterWell Home Health, you will oversee the delivery of high-quality patient care services in a home health setting. Your expertise will drive clinical operations, ensuring seamless coordination and effective management of patient care teams.
Key Responsibilities:
- Develop and implement clinical strategies to enhance patient outcomes and satisfaction.
- Lead a team of clinicians, providing guidance, coaching, and support to ensure exceptional patient care.
- Collaborate with multidisciplinary teams to develop and implement patient care plans.
- Monitor and evaluate clinical documentation, ensuring compliance with regulatory requirements and internal standards.
- Participate in performance improvement initiatives, driving continuous quality improvement and best practices.
Requirements:
- Graduate of an accredited School of Nursing.
- Two years of experience as a Registered Nurse, with at least one year in a management role in home care or hospice.
- Strong background in home health clinical operations and quality assurance.
- Experience with Home Health Care software and electronic medical records.
