Job Summary

The Case Manager plays a vital role in ensuring patients receive comprehensive care by addressing their social determinants of health needs and facilitating optimal patient outcomes. This position requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, medical providers, and other members of the interdisciplinary team.

Key Responsibilities
  • Conduct patient assessments and develop service plans to address social determinants of health needs
  • Work closely with patients' medical providers to understand their medical needs and develop plans to address them
  • Coordinate services and referrals with other members of the team to prevent gaps in care
  • Monitor patients' progress and make adjustments to service plans as needed
  • Provide education and support to patients to promote healthy behaviors and medication adherence
  • Collaborate with the interdisciplinary team to achieve optimal health outcomes for patients
Requirements
  • Bachelor's degree in a related health or human services field
  • 2 years of experience in a related field, or equivalent combination of education and experience
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Knowledge of regulations related to HIPAA and client confidentiality
  • Ability to learn and utilize client software and database
Work Environment

The Case Manager will work in multiple settings, including offices, medical clinics, and community settings. This position requires the ability to work in a fast-paced environment and adapt to changing priorities. The Case Manager may be required to work non-traditional hours, including evenings and weekends.
