Job Summary

We are seeking a highly skilled Clinical Social Worker to join our team at the Commonwealth of Virginia. As a Clinical Social Worker, you will play a critical role in providing comprehensive social work services to patients and their families.

Key Responsibilities
  • Conduct thorough assessments of patients' medical, psychosocial, and financial needs.
  • Develop and implement treatment plans to address patients' identified needs.
  • Provide supportive counseling, crisis intervention, and brief therapy to patients and their families.
  • Collaborate with interdisciplinary teams to ensure continuity of care and maximize patient outcomes.
  • Screen and briefly assess patients to identify their medical, psychosocial, and financial needs and appropriate services.
  • Participate in ongoing team huddles to ensure quality of care and level of intervention.
  • Refer patients/families with identified social work needs to MSW or LCSW social workers for comprehensive assessment.
  • Complete the Uniformed Assessment Instrument (UAI) and other government forms as appropriate to access available benefits and services for patients.
  • Arrange appropriate services and make referrals to community resources; educate patients and families regarding these resources.
Requirements
  • Bachelor's degree in Social Work from a CSWE accredited social work program required.
  • No experience required.
  • Licensure: None.
Working Conditions

The job requires sitting for prolonged periods, standing/traveling or use of assistive and climbing (stairs, steps). Proficient communicative, auditory and visual skills; Attention to detail, hear, speak, see, distinguish colors, read, ability to write legibly; Ability to lift/push/pull.
