Position Overview:
The Travel Case Manager RN plays a crucial role in the healthcare team, focusing on effective resource management and enhancing patient outcomes. This position requires a minimum of 3 years of acute care case management experience within a hospital environment.

Key Responsibilities:
  • Evaluate the medical necessity for all admissions and continued stays.
  • Identify and address any inappropriate or delayed services.
  • Assess discharge planning requirements for patients across all age groups.
  • Coordinate the delivery of necessary services to meet patient needs.
  • Facilitate timely and appropriate discharges from the healthcare facility.
  • Identify opportunities for enhancing patient care quality.
  • Document all activities as mandated by regulatory bodies.

Requirements:
  • BLS certification is mandatory.
  • Experience with discharge planning and utilization management is essential.
  • Familiarity with Cerner and MCG is required.

Work Schedule:
This role operates on a Monday to Friday schedule from 8 AM to 4:30 PM, with the expectation of availability every other weekend and rotation during major and minor holidays. Planned time off is not permitted during the contract period.
