Key Responsibilities:- Initiate, develop, implement, and revise patient care plans in collaboration with physicians and other healthcare professionals.
- Directly supervise and provide instruction to home health aides and licensed practical vocational nurses, ensuring compliance with state and federal regulations.
- Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
- Accurately and timely complete comprehensive assessments, including medication reconciliation, and document patient visits per policy and payer requirements.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
