Job Summary

We are seeking a highly skilled and compassionate Clinical Nurse Triage Specialist to join our team at Dupage Medical Group. As a key member of our telecare nursing team, you will play a critical role in supporting the remote clinical needs of our patients and providers.

Key Responsibilities
  • Handle all telecare triage phone responses, utilizing physician-approved protocols to assess and manage patient health concerns and symptoms via telephone interactions.
  • Direct patients to the appropriate level of care, which may include crisis intervention, homecare recommendations, and/or scheduling appointments.
  • Provide education and support to patients and/or family members about diagnostic procedures, medications, nutrition, and maintenance of health and wellness.
  • Utilize department-approved protocols throughout all interactions to guide patient-centered care.
  • Establish confidence with patients by acting as an advocate and seeking to resolve spoken and unspoken needs in a timely, safe, and positive manner.
  • Collaborate with team members to handle all in-basket tasks in a timely manner.
  • Act as a clinical resource for staff, providing guidance and support as needed.
Requirements
  • Active Illinois RN license and CPR certification.
  • Associate's degree in nursing required; Bachelor's degree preferred.
  • 1-2 years of nursing experience in a healthcare setting.
  • Strong clinical nursing judgment and ability to work independently.
  • Proficient in prioritization, communication, Epic EMR, typing skills, and Microsoft applications.
