Key Responsibilities
  • Handle all Telecare Triage Phone Responses, utilizing physician-approved protocols for assessment and management of patient health concerns and symptoms.
  • Direct patients to the appropriate level of care, including crisis intervention, homecare recommendations, and scheduling appointments.
  • Educate patients and/or family members about diagnostic procedures, medications, nutrition, and maintenance of health and wellness.
  • Collaborate with team members to handle all In-basket Tasks in a timely manner.
  • Act as a Clinical Resource for Staff, providing guidance and support as needed.
Requirements
  • Active Illinois RN license and CPR certification.
  • Associate Degree required; Bachelor's Degree preferred.
  • Preferred 1-2 years Nursing experience in a Healthcare setting.
