Key Responsibilities
  • Provide skilled nursing care to patients in their homes, as ordered by physicians and in accordance with individualized care plans.
  • Conduct regular assessments of patients' physical and emotional needs, and communicate findings to healthcare team members and physicians.
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Collaborate with patients, families, and healthcare team members to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
  • Experience as an LPN/LVN in a home health or acute care setting, with a strong focus on patient-centered care.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance and a clean driving record.
  • Current CPR certification.
  • Able to pass a background check and national sex offender clearance.
