Responsibilities:
  • Provide care ordered by the physician and directed by the registered nurse or appropriate skilled professional as indicated by the plan of care.
  • Assist patients with daily living activities, such as bathing, oral hygiene, nail care, foot care, skin care, and pressure relief.
  • Take accurate vital signs and demonstrate competency in required skills.
  • Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, and plans for continued care.
  • Notify licensed staff when patients complain of pain and recognize and report changes in skin condition.
  • Position patients in bed with proper body alignment and assist with ambulation and transfer techniques as necessary.
  • Perform patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Requirements:
  • Home Health Aide certificate obtained by completion of a certified home health aide course and certification by the State Department of Health Services.
  • Successful completion of a State-established or other training program that meets CMS requirements.
  • Valid driver's license and reliable transportation.
  • Proof of current CPR certification.
  • Able to pass criminal background check and national sex offender clearance.
