Substance Use Support Specialist - Emergency Care

Location: The MetroHealth System

Biweekly Hours: 80.00

Shift: Day/Night Rotation

Established in 1837, Cuyahoga County's safety-net health system operates multiple hospitals, emergency departments, and health centers.

Overview:

This role assists individuals and families affected by substance use disorders (SUD) in effectively navigating the health care landscape by eliminating obstacles to care. This includes facilitating access to essential resources, transportation, and support while improving communication with health care professionals.

Responsibilities in the emergency department encompass conducting SUD assessments, connecting patients to ongoing treatment and community resources, educating patients, liaising with clinical and administrative teams, tracking and reporting outcomes, and participating in grant-related meetings and initiatives.

Regular follow-up is conducted with the designated patient population for a duration of up to 6 weeks or longer as needed post-emergency department discharge.

Qualifications:

Required:
High School diploma or equivalent. Proficient computer skills, including familiarity with Microsoft Office applications.
Effective communication and interpersonal abilities. Capability to collaborate within a team environment.
Culturally aware and comfortable working with a diverse urban demographic. Organized and detail-oriented.
Flexibility to work varied hours, including evenings or weekends. Reliable transportation for patient home visits as necessary.
Ability to engage effectively with individuals from diverse cultural, ethnic, racial, and socioeconomic backgrounds.

Preferred:
Bachelor's degree in a relevant field. Five years of experience in health care. Background in social/human services, criminal justice, or mental health/substance abuse.
Bilingual or multilingual capabilities.

Physical Requirements:
May require intermittent movement throughout the day, including sitting, standing, bending, and walking.
May necessitate remaining stationary for extended periods, including sitting and standing.
Ability to communicate through various means, including face-to-face, phone, and email. Proficient in reading job-related documents.
Ability to operate a computer.
