  • Quality Improvement
    • Participate in the Quality Assessment and Performance Improvement (QAPI) Program, contributing to the development and implementation of quality improvement initiatives.
    • Collect and analyze clinical data to inform quality measures and improve patient outcomes.
  • Education and Training
    • Participate in regular in-services and educational conferences to stay up-to-date on best practices and Vitas policies.
    • Contribute to the orientation and training of new team members, ensuring a smooth transition into their roles.
  • Communication and Collaboration
    • Effectively communicate with patients, families, caregivers, and healthcare professionals to ensure seamless care coordination.
    • Represent Vitas Healthcare in a professional and positive manner, promoting a culture of excellence and compassion.
Requirements

A minimum of two years of experience in hospice, home health, or community health, with a strong focus on patient-centered care and quality improvement.
