Job Summary

VITAS Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as an Inpatient Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and coordinate services to ensure the best possible outcomes for our patients and their families.

Responsibilities
  • Provide comprehensive assessments and develop individualized plans of care for patients and their families.
  • Coordinate and manage patient care services, including medication management, wound care, and pain management.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
  • Stay up-to-date with the latest clinical guidelines and best practices in hospice care.
Requirements
  • Current RN licensure in the state where the VITAS program is located.
  • Minimum of two years of experience in hospice, home health, or community health.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied shifts, including evenings, weekends, and holidays.
