Prairie Hills at Cedar Rapids, a leading provider of senior living care, is seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in ensuring the health, safety, and well-being of our residents.

Responsibilities:

  • Provide hands-on care and support to residents, promoting their independence and dignity.
  • Observe, report, and document changes in resident conditions, including skin, behavior, alertness, weight, dietary, and participation in activities.
  • Respond promptly to security system and resident call bells, ensuring timely assistance.
  • Notify supervisors and/or Health Care Coordinators of increased care needs, ensuring seamless support.
  • Assist with continence management, disposing of products properly to maintain sanitation.
  • Serve and collect food trays, reporting meal refusals or loss of appetite.
  • Maintain resident rooms and work areas, adhering to cleanliness and hygiene standards.
  • Practice good standard care precautions, promoting resident safety and well-being.
  • Engage residents in life skills and other life enrichment activities, fostering a sense of community.
  • Greet and assist internal and external customers, guests, family members, residents, vendors, and team members.
  • Perform resident laundry as assigned and needed.

Medication Tech Duties:

  • Review service plans to learn pertinent resident information.
  • Assist/observe medications and treatments, using the medication observation record and the Six Rights of Medication Pass.
  • Document and initial forms as medications are given, including documentation for refusal or missed doses.
  • Maintain confidentiality of resident information, including medication.
  • Report resident concerns made while assisting with medication to the Nurse and/or Health Care Coordinator.
  • Restock medication carts after all medication passes.
  • Assist in checking medication, regardless of packaging system.
  • Count all narcotics with another Medication Care Manager or Nurse each shift.
  • Maintain and clean the Medication Room, med charts, treatment carts for neatness, cleanliness, availability of medications, and expired medications.
  • Follow the re-fill process for medications.
  • Participate in the development of the Service Plan and monthly updates.
  • Take and record temperature, blood pressure, weight, pulse, and respiration rates.
  • Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
  • Other duties as assigned by the Supervisor.

Qualifications:

  • Certified Nurse's Aid certification preferred.
  • High School diploma/GED.
  • Must be 18 years of age.
  • CPR Certification preferred.
  • First Aid Certification preferred.
  • Previous experience working with seniors preferred.
