Job Summary

We are seeking a highly skilled Patient Navigator to join our team. As a Patient Navigator, you will serve as a liaison between assigned patients and our clinical care team, ensuring that patients' expectations are met and exceeded. You will build relationships with patients and physicians, facilitating communication and implementation of a plan of care that includes appointments, transportation, education, and support services.

Key Responsibilities
  • Act as a liaison between patients and the clinical care team to ensure patient satisfaction and expectations are met.
  • Build relationships with patients and physicians to facilitate communication and implementation of a plan of care.
  • Coordinate appointments, transportation, education, and support services for patients.
  • Use independent judgment, critical thinking, and problem-solving skills to achieve the highest level of patient satisfaction.
  • Collaborate with Nurse Navigator and clinical care staff to resolve critical aspects of care that require clinical judgment and safe delivery of patient care.
Requirements
  • High School diploma or equivalent required; Associate's degree preferred.
  • 3 years of relevant work experience in customer service, preferably in healthcare.
  • Navigation/care coordination experience preferred.
  • Proficient English communication skills, written and verbal.
  • Bilingual English/Spanish preferred.
  • Proficient computer skills, including Microsoft Office Word, PowerPoint, and Excel.
