We are seeking a skilled and compassionate LPN to join our team at Maxim Healthcare Services. As a homecare nurse, you will be responsible for providing high-quality patient care and support to individuals in their homes.
Responsibilities- Assess patient needs and develop individualized care plans to meet those needs.
- Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions.
- Document patient progress and communicate effectively with healthcare providers.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Current LPN licensure in the state of practice.
- One year of prior nursing experience, preferably in home healthcare.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
