Job Overview

We are seeking experienced Nurse Practitioners to join our team as Primary Care Providers in Merrimack, New Hampshire. As a member of our team, you will conduct comprehensive health assessments, provide medical care, and collaborate with healthcare providers to ensure seamless patient care.

Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physicians on patient education and provide follow-up care.
  • Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the AANP, ANCC, or AACN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Comfortable and flexible with frequent change.
Travel Requirements
  • Able to travel, valid state driver's license, and able to drive a car with proof of adequate automobile insurance coverage for the state of residence.
  • Travel required to meet patients where they live.
Compensation

The estimated salary for this position is $105.00 per completed assessment, with flexible scheduling options available. We also offer mileage reimbursement and limited voluntary benefits.
