Job Summary

We are looking for a compassionate and experienced Physical Therapist to provide high-quality rehabilitative care to patients in their homes and communities. As a Physical Therapist with VNS Health, you will have the opportunity to work independently in the community as part of an interdisciplinary care team, delivering personalized rehabilitation therapy to patients in their homes or care facilities.

Responsibilities
  • Design and deliver personalized 1:1 rehabilitative therapy programs for patients recovering from surgery, illness, or accident
  • Assess progress by being a part of patients' day-to-day life
  • Practice independently in the community as part of an interdisciplinary care team
  • Deliver personalized rehabilitation therapy to patients in their homes or care facilities
Requirements
  • Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern
  • Licensure to practice as a Physical Therapist in New York State
