Registered Nurse Care Coordinator

We are seeking a highly skilled Registered Nurse Care Coordinator to join our team at Northeast Montana Health Services. As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of patient care services.
Key Responsibilities- Collaborate with healthcare providers and clinical teams to develop and implement effective care plans for patients with prenatal and postnatal care needs.
- Conduct pre-visit planning for care management patients, including scheduling appointments and coordinating with other members of the care team.
- Provide individual or group-based education, counseling, or self-management goal setting sessions to patients and their families.
- Review self-monitoring results and incorporate them into the medical record, assessing patient progress toward care plan and self-management goals.
- Monitor and evaluate the effectiveness of care plans, revising them as needed to reflect changing patient needs and goals.
- Follow up with patients who have not kept important referrals with outside providers, promoting patient self-management and empowering patients/families to achieve maximum levels of wellness and independence.
- Serve as a liaison to providers, patients, and families for coordination of services, interfacing with the electronic medical record (EMR) on care managed populations.
- Maintain accurate and timely documentation, participating in regular team meetings and peer review activities.
- Participate in departmental and organizational committees, as applicable, and contribute to the orientation of new personnel, precepting and mentoring peers.
- Registered Nurse Degree
- Current training and certification in Healthcare Provider Basic Life Support issued by the American Heart Association, American Red Cross, or American Health & Safety Institute
- Valid driver's license with an acceptable driving record for insurance purposes
- Experience and/or knowledge of working in Rural Health/Critical Access Hospital Settings is preferred
