Job Summary:The Registered Nurse Care Coordinator will be responsible for coordinating patient care, managing patient flow, and providing education and support to patients and their families. Key Responsibilities:- Coordinate patient care, including scheduling appointments, ordering tests, and communicating with healthcare providers.
- Manage patient flow, ensuring that patients receive timely and effective care.
- Provide education and support to patients and their families, including teaching them about their conditions, medications, and self-care.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Monitor patient progress, identifying and addressing any issues or concerns.
- Communicate effectively with patients, families, and healthcare providers, ensuring that everyone is informed and engaged in the care process.
- Registered Nurse degree.
- Current licensure as a Registered Nurse in the state of Montana.
- Experience working in a healthcare setting, preferably in a care management or case management role.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Ability to work in a fast-paced environment, with a high level of accuracy and attention to detail.
- Experience working in a rural health setting.
- Knowledge of population health management principles and practices.
- Experience with electronic medical records (EMRs) and other healthcare technology.
