About the Role

As a Registered Nurse Case Manager, you will be responsible for overseeing the implementation and ongoing assessment of patient care plans. You will work closely with our team of healthcare professionals to ensure that our clients receive the best possible care and support.

Key Responsibilities
  • Implement and assess patient care plans in collaboration with our healthcare team
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care delivery
  • Provide education and support to patients and families on proper home health care procedures
  • Work to decrease readmissions by promoting preventative care and ensuring continuity of care
Requirements

To be successful in this role, you will need:

  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to/from care sites and/or work locations
  • One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting
