We are seeking an experienced professional to fill the position of Aging Life Care Manager, a role that requires a deep understanding of older adults' needs and a passion for delivering high-quality care.

The Aging Life Care Manager will be responsible for evaluating the strengths and needs of older adults, educating clients and their families about care options, and creating personalized plans of care. This role also involves building relationships with community service providers and elder-care professionals, as well as documenting care plans and management.

Ideal candidates possess a mix of clinical expertise, understanding, and flexibility to meet the diverse needs of clients. They should be keenly aware of the cognitive, physical, mental, and emotional capacities of older adults and be able to adapt to challenging situations.

Key Responsibilities:

  • Evaluate the strengths and needs of older adults and adults living with chronic illnesses
  • Educate clients and their families/care partners about options and resources
  • Create and manage client-centered plans of care and adapt plans as needed
  • Facilitate client and family/care team meetings and relationships
  • Build relationships with community service providers and elder-care professionals
  • Document care plans and care management
  • Manage and advocate for appropriate and ongoing medical care.

Requirements:

  • Meaningful prior experience working with older adults in a clinical setting
  • Proficiency with Microsoft Office Suite, especially Word and Outlook
  • Excellent teamwork, communication, listening, leadership, and instruction skills
  • A valid driver's license and a reliable, insured vehicle
