Job Summary

Turning Point Community Programs is seeking a highly skilled and compassionate Mental Health Rehabilitation Specialist to join our team. As a key member of our staff, you will provide psychosocial, rehabilitation, and recovery-oriented services to our members, empowering them to achieve their goals and improve their overall well-being.

Key Responsibilities:

  • Provide individualized support and advocacy to clients, helping them access and receive necessary services.
  • Develop and implement individual service plans, ensuring clients receive holistic care and support.
  • Collaborate with clients, families, and care providers to ensure seamless care coordination.
  • Conduct regular home visits, job site visits, and other community-based activities to provide support and services.
  • Maintain accurate and timely records, including progress notes, activity reports, and logs.
  • Participate in team meetings and contribute to the development of program policies and procedures.

Requirements:

  • High school diploma or equivalent required; relevant higher education may be substituted for up to two years of experience.
  • Four years of experience as a provider of direct mental health services, or meeting other Mental Health Rehabilitation Specialist qualifications.
  • California driver's license and current vehicle insurance/registration.
  • Reliable means of transportation capable of passing vehicle safety inspection.
