The Hospice Aide plays a crucial role in delivering direct patient care services within a defined scope of practice. This position assists patients and nursing staff by executing nursing measures as directed by the Registered Nurse, providing essential services in the comfort of patients' homes.
Key Responsibilities:
- Engages in patient care discussions and team meetings.
- Completes a minimum of 12 hours of annual continuing education, sanctioned by the agency.
- Treats patients, healthcare providers, and colleagues with kindness, respect, and professionalism.
- Provides personal hygiene assistance, including bathing, skin care, oral hygiene, and hair care, as instructed by the RN.
- Supports patients with bathroom needs and mobility, including the use of bedpans and toilet facilities.
- Helps patients transition in and out of bed, utilizing mechanical lifting devices when necessary.
- Assists physical therapists with non-skilled treatments vital for patient rehabilitation.
- Facilitates patient mobility, including walking assistance with walkers and wheelchairs.
- Supports meal preparation and feeding as needed.
- Performs essential household tasks that contribute to the patient's health care at home, preventing or delaying the need for institutional care.
- Assists with prescribed exercises under the guidance of a qualified health professional.
- Records and monitors fluid intake and output as directed, and takes vital signs including blood pressure, pulse, and respiration rates.
- Completes daily work summaries detailing care provided to each patient.
- Reports any changes in the patient's condition, behavior, or appearance to the nurse supervisor promptly.
- Keeps organized records of individual Aide assignments for each patient.
- Submits Visit Reports as required.
- Maintains a professional appearance in accordance with agency uniform standards.
- Provides timely notification of any absences or delays without misuse.
- Adheres to infection control and safety protocols as established by the home health agency.
The role demands good physical health to fulfill the requirements of an active position, which includes extensive walking, driving, sitting, standing, and lifting. The ability to assist patients weighing 100 lbs. or more is essential.
This job description is intended to outline the primary responsibilities and requirements of the position. Additional related duties may be assigned by the immediate supervisor or management as necessary.
Qualifications:
- A minimum of one year of experience in hospice or home health care, with a solid understanding of the fundamental concepts of hospice care and the role within a patient care team.
- Possession of the appropriate certification and completion of required training hours, with competency evaluated in accordance with Federal and State regulations. LPNs with CNA certification will also be considered.
- Exhibits the emotional and mental maturity necessary for fostering positive relationships with patients, families, and colleagues.
