  • Maintain contact with patients, physicians, and clinical managers to ensure holistic care.
  • Documentation and coordination of care activities to support patient well-being.

Required Experience/Skills:

  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.

Salary Range: $44,200 - $60,900 per year

Location: This position is based in our CenterWell Home Health office.
