Job Overview

Salary: $16-$18/hourly

POSITION SUMMARY:

All Together Recovery LLC (ATR) is dedicated to providing comprehensive levels of ASAM SUD/BHSO care, which includes recovery housing, educational programs, job training, and employment assistance, along with family support services in both for-profit and non-profit settings. The Targeted Case Manager (TCM) will manage a caseload of up to twenty-five clients, ensuring they receive high-quality case management, advocacy, and access to community resources. The TCM will conduct ongoing assessments and referrals for necessary services, adhering to state and federal regulations as well as company policies.

KEY RESPONSIBILITIES AND REQUIREMENTS:

  1. Possess a valid Kentucky driver's license.
  2. Attend all required staff meetings.
  3. Demonstrate proficiency in ATR's policies and procedures, educating clients accordingly.
  4. Exhibit servant leadership qualities.
  5. Maintain integrity in all professional interactions.
  6. Communicate effectively and respond promptly to both verbal and written inquiries.
  7. Ensure a minimum of four client contacts monthly, with at least two being face-to-face, or five for clients with dual diagnoses, while maintaining a caseload of twenty-five.
  8. Collaborate with clinicians to review treatment goals, develop crisis prevention plans, and conduct strength-based assessments with clients and their families, documenting as required.
  9. Complete contact notes within 48 hours of interactions.
  10. Secure necessary legal documentation prior to service delivery.
  11. Conduct quarterly reviews of treatment goals and adjust as needed following team discussions.
  12. Stay informed about local resources available in clients' counties of residence.
  13. Understand clients' diagnoses and the challenges they pose to families, providing necessary support and interventions.
  14. Collaborate with other service providers to coordinate treatment, ensuring integration across home, school, and community settings to achieve desired outcomes.
  15. Engage in continuous improvement initiatives for organizational services.
  16. Demonstrate ethical problem-solving and conflict resolution skills.
  17. Maintain a professional demeanor at all times.
  18. Facilitate interdisciplinary team meetings as required.
  19. Provide direct care to individuals, families, and groups, including linking resources, monitoring services, advocating for client needs, and planning for discharge and follow-up, while also supporting family members when applicable.
  20. Document, review, and update treatment plans as necessary.
  21. Ensure all client contacts are made within required timeframes, with at least two being direct face-to-face interactions.
  22. Submit all required documentation and maintain client records in compliance with state and federal regulations and company policies.
  23. Ability to work both independently and as part of a team.
  24. Follow through on all tasks to completion.
  25. Work hours may vary based on client and family schedules.
  26. Reliable transportation may be necessary for travel.

Additional duties may be assigned by the clinical program manager.

EDUCATIONAL BACKGROUND:

  1. Bachelor’s degree in a human services or behavioral science field, with at least one year of relevant post-educational experience in a human services setting.
  2. Successfully complete company-approved TCM training within six weeks of employment.
  3. Ability to thrive in a fast-paced, dynamic work environment.
