Job Overview: We are seeking an experienced Registered Nurse to join our team as a Healthcare Services Manager. In this role, you will be responsible for coordinating care management services for patients across the continuum of care. This includes transitions of care, care coordination, population health and wellness interventions, and disease/chronic condition management per department guidelines.

About the Role: The successful candidate will have 5+ years of relevant clinical experience, with a strong background in case management or home care. You will work closely with patients, family members/caregivers, and healthcare providers to develop individualized care plans that meet the patient's unique needs. Your expertise will include assessment, planning, and managing member care, as well as advanced communication and interpersonal skills.

Salary and Benefits: We offer a competitive salary range of $120,000 - $150,000 per year, depending on experience. Our comprehensive benefits package includes medical, dental, and vision coverage, retirement plans, paid time off, and tuition assistance. Qualifications: To be considered for this role, you must have a current unrestricted license in the state of residence, a Bachelor's Degree in Nursing, and at least 5 years of relevant clinical experience. National certification in Case Management is desirable. Proficiency in a second language is also preferred.
