Career Overview
We are looking for a dedicated and compassionate RN/LVN to join our team as a Case Manager. In this role, you will have the opportunity to make a positive impact on the lives of our patients and their families by providing exceptional care coordination services.

Responsibilities:
• Conduct thorough assessments of patients' medical, functional, and psychosocial needs to develop personalized care plans
• Collaborate with healthcare providers, family members, and caregivers to ensure seamless transitions of care
• Coordinate referrals to specialists, home health agencies, and other community resources as needed
• Provide education and support to patients and families on disease management, self-care, and prevention strategies
• Participate in interdisciplinary care planning meetings to share expertise and insights
• Maintain accurate and timely documentation of patient interactions and outcomes in our electronic medical record system

Requirements:
• Current California RN/LVN License required
• 1-2 years of experience in acute care or case management preferred
• Strong understanding of managed care principles, including HMO policies and procedures
• Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and stakeholders
• Bilingual or multilingual skills a plus

Salary Range: $80,000 - $105,000 per year, depending on experience
