Job Title: Home Health Registered Nurse

We are seeking a compassionate and experienced Registered Nurse (RN) to provide specialized care for a young woman with a gastronomy tube (G-tube) in her home in W. Lebanon, NH.

Job Summary:

This is a unique opportunity to support the health and well-being of our client while fostering meaningful connections through reading and communication. The ideal candidate will have a valid nursing license in the State of New Hampshire, one year of working experience in the field within the last five years, and home care experience preferred.

Responsibilities:

  • Provide 1:1 care for the client, including administering medications, monitoring vital signs, and performing other nursing tasks as needed.
  • Communicate effectively with the client and her family to ensure her needs are met and her comfort is maintained.
  • Collaborate with other healthcare professionals to develop and implement a comprehensive care plan.
  • Document all care provided and maintain accurate records.

Requirements:

  • Valid Nursing License (RN) in the State of New Hampshire.
  • One year of working experience in the field within the last five years.
  • Home care experience preferred.
  • Trach & Vent experience preferred.
  • Current CPR Certification Required.
