SHARE (Supportive Housing Assistance to Reach Excellence) is dedicated to providing essential supportive housing services aimed at assisting individuals with significant mental health challenges in achieving and sustaining independent living within the community. Our case managers play a crucial role in delivering the necessary support within the clients' home environments and the broader community.

Position Title: SHARE Case Manager
Position Type: Full-time
Benefits Eligible: Yes
Department: Share/Adult Residential Services
Compensation: $17.00 Hourly

Key Responsibilities:

  • Facilitate therapeutic rehabilitative skill enhancement.
  • Contribute to the development of the Comprehensive Recovery Needs Assessment (CRNA) and Individual Recovery Plan (IRP).
  • Coordinate Activities of Daily Living (ADLs) for clients, under supervision, to promote independent community living.

Qualifications:

  • A Bachelor's Degree in a Behavioral Health Science field is required.
