Job Title: Registered Nurse

TexMed Home Health is seeking a compassionate and skilled Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in providing exceptional care to our clients in all stages of life.

Job Summary:

We are looking for a Registered Nurse who is experienced in nursing, with an emphasis on community health education and experience. The successful candidate will be responsible for planning, organizing, and directing home care services, and will work closely with our interdisciplinary team to ensure the best possible outcomes for our clients.

Key Responsibilities:
  • Conduct initial home health assessments of patients and families to determine home care needs
  • Provide comprehensive and ongoing assessments of patients and families to determine hospice needs
  • Conduct home visits as assigned, assessing the patient's condition, and progress toward goals, providing patient care, teaching, and other skilled interventions as ordered
  • Regularly re-evaluate patient nursing needs, as part of the nursing visits
  • Initiate the plan of care and make necessary revisions as patient status and needs change in collaboration with the Case Manager
  • Use health assessment data to determine nursing diagnosis
  • Develop a care plan in collaboration with the Case Manager, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions
  • Initiate appropriate preventive and rehabilitative nursing procedures
  • Counsel the patient and family in meeting nursing and related needs
  • Provide health care instructions to the patient as appropriate per assessment and plan of care
  • Identify discharge planning needs during nursing visits and communicate needs to the team as part of the care plan development
Requirements:
  • Registered nurse with current licensure to practice professional nursing in the state
  • Graduate of the National League for the Nursing accredited school of nursing
  • Maintains a current CPR certification
  • Current valid driver's license
  • Minimum of two years experience, at least one of which is in the area of public health, and home care is preferred
  • Self-directed and able to work with minimal supervision
  • Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist
  • Shows ability to organize and prioritize workload independently
Work Environment:

The Registered Nurse will work in a fast-paced home health environment, providing care to patients in their homes. The ideal candidate will be able to work independently and as part of a team, and will have excellent communication and problem-solving skills.
