We are seeking an experienced APRN to join our team in the Oncology Long-Term Follow-Up Clinic. The successful candidate will provide high-quality patient care, education, and support to patients with breast cancer.
Responsibilities- Provide comprehensive patient care, including assessment, diagnosis, and treatment planning, in collaboration with the attending physician.
- Develop and implement individualized treatment plans, taking into account patient preferences and needs.
- Conduct thorough medical histories, perform physical examinations, and order diagnostic tests as necessary.
- Prescribe medications and treatments, as authorized by state law and hospital regulations.
- Collaborate with the multidisciplinary team to ensure seamless care coordination and communication.
- Provide patient education and support, including counseling on treatment options, side effects, and follow-up care.
- Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines.
- Graduate from an accredited Nurse Practitioner or Physician Assistant program.
- Active certification as a PA or NP, with a focus on oncology.
- Current licensure as a PA or NP in the state of Connecticut.
- Experience working in an oncology setting, preferably in long-term follow-up care.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
