As a Licensed Practical Nurse (LPN) at Dartmouth Health, you will provide high-quality nursing care to patients and families within the scope of practice of the LPN license as defined by the State of New Hampshire Board of Nursing.

Key Responsibilities:
  • Provide nursing services to patients and families, including gathering information from patients and/or caregivers, and providing telephone nursing care.
  • Meet or exceed standards of performance as established for employment at DHK, with a strong focus on exceptional customer service.
  • Efficiently gather information from patients and/or caregivers in person or on the phone, and provide telephone nursing care, using departmental guidelines, DHK standard telephone triage books, and provider-specific information.
  • Recognize your limits of knowledge and experience, and plan for management of situations beyond your expertise, using provider and RN available resources in the department.
  • Review medical records of scheduled patients to verify that current lab, x-ray, or procedure reports are available for provider, and fill out forms required for appointment.
  • Accurately perform required computer functions, including medication reconciliation, updating past medical and surgical history problem lists in the EMR, chronic health maintenance, scheduling of appointments, including order entry, and management of task lists.
  • Escort patients to exam rooms and prepare for examination/procedure, aware of age, language barriers, and physical limitations.
  • Assist providers with exams and procedures as necessary.
  • Prepare, administer, document, and submit charges for immunizations or medications and treatments under provider direction.
  • Send lab specimens to the lab with all necessary paperwork or computerized order entry completed, and the specimens properly labeled.
  • Point of Care testing obtained, documented, and communicated to appropriate providers.
  • Accurately document patient interactions, including but not limited to calling or transmitting of prescriptions, notification of test results, and nursing advice.
  • Obtain prior authorization or pre-certification as required.
Requirements:
  • Graduate of an accredited practical nursing program.
  • 0-2 years of experience as a LPN required.
  • Must maintain NH LPN License.
  • BLS within 3 months of hire and maintained on a bi-annual basis.
Work Status:

Monday - Friday, 7:45am to 5:00pm, with one day off during the week and rotating Saturday am clinic.
