  • Provide Nutritional Counseling and Education: Offer expert nutritional guidance and education to patients and their families, promoting healthy eating habits and managing medical conditions through diet.
  • Review and Submit Medically Tailored Meal Referrals: Review and submit all medically tailored meal referrals across CalAIM to ensure patients receive the necessary nutrition support.
  • Collaborate with Healthcare Professionals: Work closely with physicians, nurses, and other healthcare professionals to develop and implement patient care plans that incorporate nutritional therapy.
  • Monitor and Evaluate Patient Progress: Regularly monitor and evaluate patient progress, adjusting nutrition plans as necessary to achieve desired outcomes.
  • Stay Up-to-Date with the Latest Research: Stay current with the latest research and developments in the field of nutrition, ensuring that all advice and plans are based on current evidence-based practices.
  • Document Patient Interactions: Document all patient interactions, assessments, and plans in the appropriate medical records.
  • Participate in Nutrition-Related Initiatives: Participate in the development and implementation of nutrition-related policies and procedures, as well as conduct group educational sessions and workshops on nutrition and healthy living.
  • Support Community Outreach Programs: Assist in the planning and execution of community outreach programs related to nutrition and wellness.
Requirements:
  • Education: Bachelor's degree in Nutrition, Dietetics, or a related field.
  • Credentials: Registered Dietician (RD) or Registered Dietician Nutritionist (RDN) credential from the Commission on Dietetic Registration (CDR).
  • Licensure: Licensed to practice as a Dietician in California.
  • Language Skills: Bilingual in English and Spanish.
  • Work Environment: Hybrid position, requiring remote work and occasional travel to Titanium Healthcare's Garden Grove office.
  • Experience: Minimum of 2 years of experience in clinical nutrition, preferably in a healthcare setting.
  • Desirable Skills: Experience working with CalAIM or similar programs, strong knowledge of medical nutrition therapy for various conditions, excellent communication and interpersonal skills, and ability to work effectively in a multidisciplinary team.
