Clinical Director, Pre-Enrollment Operations

Habitat Health is a pioneering healthcare organization dedicated to delivering comprehensive care to older adults. We are seeking a highly skilled Clinical Director, Pre-Enrollment Operations to join our team.
Key Responsibilities- Define Clinical Assessment Process: Develop and implement a robust clinical assessment process for prospective PACE participants, ensuring accurate identification of level of care and health & safety criteria.
- Collaborate with Marketing Teams: Work closely with marketing teams to ensure clinically informed and accurate messaging.
- Standardize Pre-Enrollment Processes: Establish and operationalize Standard Operating Procedures (SOPs), committees, and related workflows to oversee prospective participant IDT/H&S denials.
- Medical Record Review: Develop and implement a medical record review workflow to inform the process of establishing eligibility for PACE.
- Establish Key Metrics: Develop and track key clinical pre-enrollment metrics to measure success.
- Support Pre-Enrollment Strategy: Collaborate with the team to develop, refine, and implement pre-enrollment strategy and related clinical initiatives.
- Provide Training and Support: Develop job aids, training materials, and provide coaching to Habitat Health nursing staff involved in the pre-enrollment process.
- Education: Bachelor's degree in nursing (BSN) preferred; professional experience may be substituted.
- Experience: 7+ years of relevant experience in healthcare delivery, patient growth, and community outreach; prior experience at a PACE organization is strongly preferred.
- Skills: Excellent analytical and project management skills, strong interpersonal and written/oral communication skills, and a strong passion for our mission.
Habitat Health offers a competitive salary range of $150,000 - $175,000, bonus-eligible, and a comprehensive benefits package, including medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays.
