Our Commitment
This role requires a dedicated Practical Nursing Specialist who will join a Clinical Care Team, collaborating closely with Nurse Practitioners. The team will oversee the health management of a designated patient population in conjunction with the patients' primary care and specialist providers. You will engage with your community through daily travel to patients' residences, fostering meaningful relationships through in-home visits.
The ideal candidate will be proficient with mobile technology and possess a strong commitment to outstanding patient care. A typical workday may involve conducting clinical assessments at patients' homes, performing telephonic evaluations, providing patient education, ensuring adherence to treatment plans, connecting patients with community resources, executing routine diagnostic tests, and documenting assessments in Spiras systems. This position offers both personal and professional fulfillment, allowing you to deliver healthcare services at the forefront of your training. The role combines working from a home office with community visits, providing flexibility in work hours while maintaining a focus on patient-centered care.
Key Responsibilities
- Conduct comprehensive patient assessments, including vital signs, monitor changes in health status, report findings, contribute to and implement care plans, collaborate with Nurse Practitioners, and provide patient education.
- Assist patients in achieving treatment goals and identify barriers to care.
- Evaluate treatment effectiveness by observing and documenting patient progress, recommending necessary adjustments.
- Communicate effectively with physicians, nurses, social workers, family members, caregivers, and other healthcare professionals.
- Recommend assistive devices and follow-up programs for outpatient or home health care.
- Ensure accurate and timely documentation of patient care in accordance with policies.
- Adhere to hygiene and safety protocols consistently.
- Foster a supportive and compassionate environment for patients and their families.
- Comply with established policies, procedures, and standards.
- Other duties as assigned.
- As part of a market team, you may be required to cover patients within the designated market area, typically within a 60-mile radius of your home address. Scheduling procedures are designed to minimize travel time and enhance patient care delivery.
- Additional travel may be necessary.
- Completion of an accredited LPN program.
- Minimum of 2 years of experience as a Licensed Practical Nurse.
- Strong knowledge of nursing care practices and procedures.
- Familiarity with emergency care protocols.
- Active and valid state nursing license.
- Reliable and insured personal vehicle.
- Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.
- Experience in cardiac or pulmonology specialties.
- Knowledge of advance directives and palliative care.
- Experience with clinically complex geriatric patients.
- Bilingual (English/Spanish).
- Experience in home health care.
- Strong verbal and written communication skills.
- Able to quickly gather and assess necessary information for patient care.
- Comfortable working with patients who may have hearing difficulties, memory issues, or lower health literacy.
- Desire to collaborate with healthcare team members.
- Ability to think critically and provide information with confidence.
- Strong organizational skills to manage competing priorities and meet deadlines.
