Key Responsibilities
  • Develop and Implement Care Plans: Create and execute personalized care plans in accordance with patient needs and medical requirements.
  • Provide Skilled Nursing Care: Administer medication, perform wound care, and provide treatments and procedures as needed.
  • Monitor Patient Conditions: Regularly assess and report changes in patient conditions to Clinical or Client Services Manager.
  • Document Patient Care: Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
  • Collaborate with Healthcare Team: Work closely with doctors, therapists, and other healthcare professionals to ensure comprehensive patient care.
Requirements
  • Current RN License in Vermont: Possess a valid and up-to-date license as a Registered Nurse in Vermont.
  • Minimum 1 Year of Clinical Experience: Have a minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Home Care Experience Preferred: Prior home care experience is strongly preferred but not required.
  • Strong Interpersonal Skills: Possess excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
