Responsibilities and Expectations- Care Planning and Delivery: Develop and implement personalized care plans for clients, ensuring their unique needs are met with compassion and professionalism.
- Daily Living Assistance: Assist clients with daily living activities, such as bathing, dressing, and grooming, promoting independence and dignity.
- Meal Preparation: Prepare healthy meals and snacks, considering dietary restrictions and preferences.
- Household Maintenance: Maintain a safe and clean living environment, performing light housekeeping duties and laundry.
- Medication Administration: Administer medications and provide reminders, ensuring timely and accurate administration.
- Documentation and Communication: Document all services provided, maintaining accurate records and reporting any changes or concerns to supervisors. Communicate effectively with clients, families, and healthcare professionals, fostering strong relationships and ensuring seamless care coordination.
To be successful in this role, you'll need:
- A valid HCA registration with the State of California Department of Social Services.
- Minimum 3 months of experience in home care or a related field.
- Excellent communication and interpersonal skills.
- Able to lift 25 pounds and perform physical tasks as required.
