Palliative Care Specialist

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team in Washtenaw County. As a key member of our healthcare team, you will provide compassionate, patient-centered care to individuals in their homes.
Job Description:- Provide palliative care services to patients and families in their homes, focusing on symptom management, pain control, and improving quality of life.
- Work collaboratively with palliative care physicians and other healthcare professionals to develop and implement individualized care plans.
- Conduct comprehensive assessments, including physical exams, medical histories, and laboratory tests to identify health issues and develop treatment plans.
- Prescribe medications, order diagnostic tests, and provide education to patients and families on disease management and self-care techniques.
- Masters degree in Nursing with national certification as a Nurse Practitioner.
- Active license to practice as a Nurse Practitioner in the state of employment.
- DEA License (required).
- Board certification in Palliative Care (preferred).
- At least 3 years of clinical nursing experience, with a minimum of 1 year in hospice or palliative care (preferred).
