Job Overview:

As a vital member of the healthcare team, the Ambulatory Care Pharmacist delivers comprehensive pharmaceutical care services in collaboration with other healthcare professionals. This role involves managing patient medication regimens, educating patients, and overseeing the preparation and dispensing of medications. The focus is on promoting high-quality, evidence-based, and cost-effective pharmaceutical care within assigned clinical areas.

Key Responsibilities:
  • Adhere to Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and relevant laws.
  • Conduct thorough medication history assessments based on referrals from healthcare providers.
  • Develop and implement treatment plans that may encompass pharmacotherapy and lifestyle adjustments, ensuring seamless transitions across care settings.
  • Identify, address, and avert medication-related issues, including untreated conditions, inappropriate drug choices, dosage errors, and adverse drug reactions.
  • Initiate, modify, monitor, and recommend adjustments to drug therapies in accordance with established protocols and patient assessments.
  • Coordinate laboratory monitoring of drug therapies, including serum drug levels and other relevant tests.
  • Monitor patient outcomes to optimize drug utilization, compliance, and minimize adverse drug reactions.
  • Provide patient education on disease management, medication adherence, and potential side effects.
  • Engage in continuous improvement of pharmaceutical care services.
  • Deliver educational sessions to healthcare providers and staff through various formats.
  • Conduct drug utilization reviews and medication reconciliations.
  • Utilize pharmacy information systems and maintain accurate documentation in patient records.
  • Support a collaborative work environment.
  • Supervise ancillary staff, including pharmacy technicians and interns.
  • Perform additional duties as required.
Qualifications:

Experience:
  • At least 1200 hours of experience in collaborative drug therapy management within the last five years, or completion of an ASHP Accredited PGY-1 residency.
  • Must possess one of the following credentials: Board Certified Oncology Pharmacist (BCOP), completion of a PGY-2 oncology residency, or significant experience in sterile compounding oncology pharmacy.
Education:
  • Pharmacy degree meeting the educational requirements for licensure.
Licensure and Certification:
  • California Pharmacist License required upon hire.
  • Basic Life Support certification within three months of hire.
  • National Provider Identifier required upon hire.
Additional Skills:
  • Passage of a standardized competency assessment.
  • In-depth knowledge of pharmacological principles, medical terminology, and disease management.
  • Familiarity with federal and state pharmacy regulations.
  • Experience with aseptic techniques and preparation of sterile products.
  • Ability to extract and analyze relevant patient information for care planning.
  • Strong communication and teaching skills.
  • Proven analytical and problem-solving abilities.
  • Experience in a multidisciplinary healthcare team setting.
Preferred Qualifications:
  • Doctorate of Pharmacy (Pharm.D.).
  • Completion of a PGY-2 clinical pharmacy specialty residency.
  • Board certification in a relevant practice area.
  • Advanced Practice Pharmacist License.
