Job Description

We are seeking a Licensed RN in Massachusetts to work in the community with adults experiencing severe and persistent mental illnesses. As a primary case manager, you will provide nursing interventions, assist with medication management, and develop Comprehensive Assessments and Individualized Action Plans for clients on your caseload.


Responsibilities
  1. Act as a primary case manager to 3-6 consumers
  2. Provide nursing interventions to individuals
  3. Assist in the management of medications and the overall functioning of the nursing department
  4. Formulate and regularly update Comprehensive Assessments and IAPs for clients on your caseload
  5. Participate in supervision, staff meetings, in-services, and trainings
  6. Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed
