As a Home Care Clinician, you will work collaboratively with a multidisciplinary team to develop and implement individualized care plans that promote optimal clinical outcomes and patient independence.
Key Responsibilities:- Develop and document individualized care plans with specific interventions and measurable patient outcomes.
- Evaluate progress toward goals and revise care plans as needed.
- Establish and implement personalized teaching plans for patients and their caregivers to promote independence at home.
- Perform nursing procedures consistent with homecare protocols and utilize standard homecare supplies and equipment.
- Initiate and participate in regular case conferences with the patient's physician, home healthcare team, and other key members of the care team.
- Maintain accurate clinical records, collect data, and prepare reports on activities.
- RN residency programs and tuition assistance to support your career advancement.
