Hospice Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team at Haven Hospice. As a Registered Nurse, you will play a vital role in providing high-quality care to patients and their families in the comfort of their own homes.
Responsibilities:- Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
- Provide expert nursing care, including administering medications, treatments, and performing various medical procedures as prescribed by physicians.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Teach patients and families about self-care techniques, medication management, and other essential skills to promote independence and comfort.
- Document patient progress, pain management, and other relevant information in a timely and accurate manner.
- Communicate effectively with patients, families, and healthcare professionals to ensure optimal care outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance patient care.
- Current Registered Nurse licensure in the state and American Heart Association CPR certification.
- Graduate of an accredited school of nursing with a Bachelor's degree preferred.
- One year of hospice experience preferred, but not required.
- Excellent observation, verbal, and written communication skills, problem-solving skills, and basic math skills.
