About the Role:
Catholic Charities of the Diocese of Albany is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring that our clients receive the highest level of care and support.
Key Responsibilities:
- Assess and develop care plans to address gaps in care and social determinants of health
- Coordinate the care of clients with two or more chronic illnesses
- Refer and link clients to needed services and resources
- Work with high-risk populations, including clients with mental health diagnoses, homelessness, HIV/AIDS, Sickle Cell Anemia, and/or Substance Use Disorder
- Ability to work in a fast-paced environment
- Complete proper documentation
Requirements:
- Bachelor's Degree (preferred but not required) in Health, Education, or Human Services
- 1-3 years of case management experience
- Experience working with diverse populations
