About the Role

We are seeking a highly skilled Healthcare Clinical Navigator to join our team at Roxborough Memorial Hospital. As a Healthcare Clinical Navigator, you will be responsible for developing and implementing effective care navigation strategies to ensure seamless patient transitions across the healthcare continuum.

About You

We are looking for a dedicated and compassionate individual with a minimum of 5 years of work experience post-graduation in an accredited school of nursing and a current state Registered Nurse license. Your expertise in patient navigation, care coordination, and communication will enable you to effectively collaborate with healthcare providers to achieve optimal patient outcomes.

Responsibilities

  • Develop and implement effective care navigation plans that address patient needs and preferences.
  • Collaborate with healthcare teams to coordinate patient care services, including laboratory, radiology, and pharmacy services.
  • Identify opportunities for process improvements and implement changes to enhance patient care and workflow efficiency.

Requirements

  • Bachelor's degree in Nursing from an accredited institution.
  • Minimum 5 years of acute care nursing experience preferred.
  • At least one year experience in case management, discharge planning, or nursing management preferred.
  • Current BCLS certificate preferred.

Compensation and Benefits

The estimated annual salary for this role is $90,000 - $115,000, depending on experience. We offer a comprehensive benefits package, including medical, dental, and vision insurance, retirement savings plan, paid time off, and continuing education opportunities.
