About the Role

We are seeking a skilled and compassionate Senior Home Health RN to join our team at Providence Home and Community Care. As a Senior Home Health RN, you will provide high-quality care to patients in their homes, working under the direction of a physician.

Key Responsibilities
  • Provide quality care to patients in their homes, assessing their needs and developing individualized care plans.
  • Coordinate patient care activities with other members of the healthcare team, including physicians, therapists, and social workers.
  • Collaborate with patients and their families to ensure that their needs are met and that they receive the best possible care.
  • Document patient care activities and outcomes in a timely and accurate manner.
Requirements
  • Bachelor's Degree in Nursing
  • Washington Registered Nurse License
  • National Provider BLS - American Heart Association
  • National Provider Identifier (NPI) number
  • Certified Wound Ostomy Continence Nurse (CWOCN)
  • 3 years of Nursing experience
Preferred Qualifications
  • Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience
