About the Role

We are seeking a skilled and compassionate Senior Home Health RN to join our team at Providence Home and Community Care. As a Senior Home Health RN, you will provide high-quality care to patients in their homes, working under the direction of a physician and collaborating with other healthcare professionals to coordinate patient care activities.

Responsibilities
  • Provide quality care to patients in their homes, including wound care, ostomy care, and continence care.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care for patients.
  • Communicate effectively with patients, families, and caregivers to ensure that care plans are understood and implemented.
  • Document patient care activities, including assessments, care plans, and progress notes, in accordance with regulatory requirements.
Requirements
  • Bachelor's Degree in Nursing
  • Washington Registered Nurse License
  • National Provider BLS - American Heart Association
  • National Provider Identifier (NPI) number
  • Certified Wound Ostomy Continence Nurse (CWOCN)
  • 3 years of Nursing experience
Preferred Qualifications
  • Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience
