Key Responsibilities:- Evaluate patients to identify their needs and develop personalized therapy plans.
- Follow up with patients to monitor their progress and adjust treatment plans as needed.
- Provide occupational therapy services to patients in their homes, focusing on activities of daily living, home safety, and equipment evaluation.
- CA Licensed Physical Therapist (PT) or Certified Occupational Therapist Assistant (COTA).
- CPR Certified and recent physical within the last year.
- Clear TB Testing within the last year and excellent communication skills.
- Experience working with geriatric patients and a strong understanding of occupational therapy principles.
