Job Summary

We are seeking a highly skilled and compassionate Licensed Master Social Worker to join our Neuro Rehabilitation team. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care to individuals with complex needs.

Key Responsibilities
  • Develop and implement comprehensive social work care plans to support individuals in achieving their goals and improving their quality of life.
  • Collaborate with the treatment team to ensure seamless care coordination and timely discharge planning.
  • Provide emotional support and advocacy to individuals and their families, promoting their rights and benefits.
  • Facilitate real-time communication with external stakeholders, including insurance adjustors and external physicians.
Requirements
  • Master's degree in Social Work
  • Three years of case management experience or related social work experience and full license (LMSW)
